Home Care: Assistance with daily activities at home
Therapies: Physical, occupational therapy as needed
Transportation: Rides to and from medical appointments and the PACE center
Medications & Supplies: Prescription drugs and durable medical equipment

ELIGIBILITY

To enroll in K-day, you need to:

Be 55+ years
Reside in K-day’s service area (check below zip code)
Require nursing facility level of care, as defined by California
Be able to live safely in the community with the help of PACE services at the time you enroll.
